Vacation Policy:

Staff members requesting vacation/time off must give at least one week of notice for less
than two days off. If the request for time off is a week or more, a minimum of six weeks’
notice is needed.

Anyone requesting vacation/time off beyond a two month window from the date of
the written request will be considered for time off. This is on a “first come first serve”
basis, as two employees with the same job title cannot be off at the same time.

Maternity Leave:
A maternity leave of up to six weeks is permitted. Paid vacation and sick days may
be used in addition to unpaid leave. No other time off is allowed in the same year.

Sick Leave:

It is your responsibility to call the practice manager or head dental assistant if you are sick. The sick employee must give notice as soon as possible so a replacement can be found. An absence of 2 days or longer requires a doctor’s note. Abuse of this policy will
not be tolerated and can be ground for dismissal.
